HOW DO I....

Obtain A Repeat Prescription?

For patients needing regular long-term medication we have a computerised system of repeat prescriptions.

Please leave the computer list, marking clearly all the items you need, in the box in reception or post it to us enclosing a S.A.E.

Please allow at least 48 hours for processing. The completed prescription will be available at reception. Try to organise your request to once a month. It is important that your treatment is monitored regularly, so every three to six months you will find a reminder on your prescription asking you to make an appointment with the doctor.

Register?

New Patients living in Abercynon and Mountain Ash can register with the practice as follows:

a) If you have a medical card, fill in your new address details where applicable

b) If you do not have a medical card, ask at reception for a GMS1 form. Fill in your details on the form.

You will be asked to make an appointment first with the practice nurse for a general health check and your urine sample with you.

You should bring a sample of urine with you to this appointment.

For patients who express a preference of doctor this can be arranged by appointment.

Patients who require an emergency appointment ask at reception and these will be seen by whichever doctor is available on the day.

Children who are sick and need to see the doctor will be seen the same day.
